We're using Prefect Cloud + Work Pools so I don't have the same visibility into the underlying logs but we had an outage caused by duplicate ECS tasks spun up during the same Flow Run. Their start times in ECS was about 30s apart and the logs from both tasks were tracked in the same Flow Run.
